Form of payments:
We accept cash, Visa, MasterCard, Discover, American Express, and Checks. All returned checks will be assessed a minimum $50.00 fee to the patient's account.
Non-insured patients:
Payment in full is required on the day services are rendered unless prior arrangements had been made.
Insured patients:
Any co-payments will be estimated by our office and is to be paid in full at the time of service. If the patient does not agree to the specified co-payment, then D & L Dental Center will not accept the patient's insurance assignments of benefits. The patient shall pay the entire cost of treatment and submit his or her own dental claim. There are no exceptions.
It is not the responsibility of D & L Dental Center for collecting on any outstanding insurance claims or to negotiate a settlement on a disputed claim as this is solely the patient's responsibility. After 60 days from the treatment date, any dental claims unpaid or denied will be due in full from the patient or the responsible party.
D & L Dental Center uses a third party billing service, First Pacific Corporation. Every month, patients will receive a statement requiring a small minimum payment on the outstanding balance. This minimal monthly payment is required even if a co-payment was paid at the time services were rendered. If this minimum payment is made, the patient will not incur any late fees or interest charges.
Emergency patients:
If insurance is not verifiable at the time of service, then full payment is expected at the time of service.
Financing & Collections:
Interest-free financing is available. Please ask for details.
After 60 days from the date of treatment, all unpaid balances on patient's accounts are due in full, irregardless of insurance coverage. If an account is sent to collection, 35% of the unpaid balance will be added to the outstanding balance as collection fees.
Cancellations and No-shows:
There is a minimum $50.00 fee charged to the patient's account without a 24 hour cancellation notice. For larger treatment cases, (above $500.00), the non-cancellation fee will be 10% of the treatment for the missed appointment.
