The membership fee for 12 consecutive months for a child or adult will entitle the individual to the services as listed. If treatment is needed after the D & L Dental Plan is administered, the individual is entitled to 10% discount on all D & L Dental’s regular fees.
The D & L Dental Plan are for patients with no insurance coverage. This plan cannot be used in conjunction or separately with any insurance plans or with any other in-office discount programs.
